I think this is very creative. While the idea itself is not new, and is done almost every year in journals around dA, this is the first time I've ever seen it done in deviation form, and all the filled out forms I've seen are stunning.

I love the consistency throughtout the meme, it's not too busy and distracting from the art. The colours are kept similar, the boxes are evenly spaced and the same size, and overall, it's very neat, so it serves it's purpose excellently without become too noticeable. Also, the colour choice is very nice for that purpose, and the lack of too many decorations or details is very unassuming and allows the artist to do what they want with it and decorate it to their tastes. Again, very good :thumbsup: The writing style is legible and appealing, and it too, is a good choice.

There is only one addition I feel should be made to this. One more box, perhaps a little bigger or better and more special, for the favorite deviation of the entire year. on the top or the bottom. I think that would round off the meme perfectly ^^

As far as Convenience goes this is top notch, a truly Grand idea. The complementing colors and font bring a professional yet fun and free feeling to the deviation. The blue border looks great with the gray foreground. I especially like how "2009" is also blue. There is just enough space between each item and word, perfect "white Space".

As far as Originality goes, I have seen some meme's like this before in the past. But yours is one of the best I have seen.

I can tell you right now that I intend to use this and post my own on my page.
